Contribution Remittance Form
If you have lost your remittance form and wish to send in a
deposit to your account, please print this form to send along with your
check.
Direct Rollover Request
A form to be completed in order to rollover funds from another
financial institution into your Board of Pensions account.
Salary Reduction Agreement
A contractual form to be completed by you and your employer and
kept on file by your employer, if your retirement contributions are the
result of a salary reduction (member before-tax contributions).
Membership Application Form
If you are an eligible Church of God employee, this is the form
to submit along with your first deposit in order to open a retirement
account.
Statement of Estimated Benefits
A form to request a calculation of what your estimated benefit
would be at retirement, based on your current account accumulation plus
any future deposits and estimated earnings.
